Erliz Maldonado, MSN, APRN, FNP-C, is a certified Family Nurse Practitioner at Core Primary Care. Erliz provides personalized care to all patients from two offices in Sugar Land and Houston, Texas.

Erliz is a proud Texas native. She began her nursing career 11 years ago as a Licensed Vocational Nurse. She earned her Licensed Vocational Nursing certificate at Wharton County Jr. College in 2010. 

In 2014 her Bachelor of Science in Nursing degree at Prairie View A&M University College of Nursing at Houston, and she completed her Master of Science in Nursing degree at The University of Texas Health Science Center at Houston-Cizik School of Nursing (Cizik School of Nursing at UTHealth Houston).

Erliz has cared for many patients through various nursing positions throughout her nursing career and has worked at Ben Taub General Hospital and Houston Methodist Hospital.

After flourishing in her career, she decided to increase her medical knowledge by furthering her education to help her patients achieve their best quality of life.

She is fluent in Spanish and English and passionate about helping our patients prevent and care for chronic and acute illnesses.

In her free time, Erliz enjoys spending quality time with her daughter, fiancé, family, friends, and her two lovely dogs, Lola (Goldendoodle) and Toby (German Shepherd). She also loves traveling, especially in sunny areas next to a beach, and enjoys eating different kinds of sushi.

Mimi Chandrabose, PA-C, is essential to the Core Primary Care team, bringing over 13 years of adult medicine experience to support the needs of patients. Mimi possesses advanced clinical knowledge that aids in supplying life-saving care. Mimi earned a bachelor’s in science, biology, and biomedical science from Houston Baptist University. She followed up with a master’s in medical science and physician assistant studies from Methodist University in Fayetteville, North Carolina. Her time in higher education set the foundational knowledge that is the basis for her clinical practice. 

Following her master’s, Mimi built upon that foundational knowledge by working as a physician assistant for the Le Family Practice and Health Center in Houston, Texas, where she still serves today. She also worked for the Diabetes Metabolic Center in Houston, Texas, where she treated patients with metabolic disorders, such as diabetes, and those experiencing thyroid difficulties. 

Mimi continued to broaden her scope of practice and was a leading physician assistant for the Houston Gastrointestinal and Liver Clinic in Sugarland, Texas. Here, she flawlessly executed various gastrointestinal and liver treatments. Mimi performed diagnostic screenings, interpreted test results, and managed workflows with patient scheduling and prescription refills.

Mimi is board-certified by the National Commission on Certification of Physician Assistants. She received the BLS and First Aid Certified awards for her passion and excellence in delivering quality health care. Nurse Practitioner

Sheila Esparza, APRN, is an esteemed, advanced nurse practitioner serving the Core Primary Care communities in Sugarland and Houston, Texas. Sheila's patient-focused expertise puts the patient first to address any concern and implement an appropriate medical intervention while ensuring comfort. 

Sheila started her nursing journey with an associate degree from Houston Community College. She gained essential hands-on experience as a youth care worker helping adolescent populations with mental health and maintaining prescribed routines, such as physical therapy and medications.

Sheila earned a bachelor’s in nursing from Prairie View A&M University in Prairie View, Texas. During this time, Sheila continued to hone her nursing skills and worked as a registered and clinical nurse for various Houston hospital systems, such as the Lyndon B. Johnson Hospital.

Returning to higher education, Shiela received a master’s degree from Chamberlain University in Cypress, Texas, to become a family nurse practitioner. She continues to care for in-need populations by performing thorough visual exams, educating patients on their health, and minimizing the risk of future potential complications.

In addition to treating several illnesses and injuries, Sheila offers annual physicals, prescription refills, and contraception advice. Sheila brings her passion and experience to Core Primary Care to uplift the Houston and Sugarland, Texas communities. She is also fluent in Spanish and maintains her BLS and ACLS certifications.
